Where and when to submit your application
The application for registration of stay is submitted in person to the voivode competent for the place of stay of the EU citizen, no later than on the day following the expiry of 3 months from the date of entry into the territory of the Republic of Poland. The requirement to submit an application in person does not apply to a minor.
The obligation to register the stay does not apply to an EU citizen who has entered the territory of Poland in order to look for a job and who may stay in Poland without the need to meet the conditions of stay for a period not longer than 6 months or for a longer period, if he proves that he is actively pursuing job search and has real employment opportunities.
Required documents:
Completed application form for registering the stay of a citizen of the European Union;
A valid travel document or other valid document proving his identity and citizenship. (original available for inspection);
Note: Lack of any of the above the documents will result in the call of the foreigner to its supplementation within 7 days from the delivery of the request under the pain of leaving the application without examination.
Certificate of registration of residence of a European Union citizen, issued to a European Union citizen with whom the family member resides on the territory of the Republic of Poland;
Other required documents:
a) in the case of an employee:
– a written statement of the employer or entity authorized to entrust the European Union citizen with the performance of work on the intention to entrust him with work or a certificate of performance of work or
b) in the case of a self-employed person in Poland:
– a written declaration regarding the entry in the National Court Register or the Central Register and Information on Economic Activity or any other proof that the citizen of the European Union is a self-employed person in the territory of the Republic of Poland;
(c) in the case of students or persons undergoing vocational training:
– certificate of a state or non-state higher education institution on admission to studies or a referral for vocational training,
– a document confirming coverage by universal health insurance or the right to use health care services on the basis of the provisions on the coordination of social security systems or the possession of private health insurance,
– a written declaration of having sufficient financial resources to support oneself and family members, so as not to constitute a burden for social assistance, or a proof of their possession;
d) in the case of a spouse of a Polish citizen:
– a document confirming marriage with a Polish citizen,
e) in other cases:
– a document confirming coverage by universal health insurance or the right to use health care services on the basis of the provisions on the coordination of social security systems or the possession of private health insurance,
– proof of having sufficient financial resources to support oneself and family members, so as not to constitute a burden for social assistance.
Evidence of having sufficient financial resources to support oneself and family members, so as not to constitute a burden for social assistance, may include in particular:
1) credit card;
2) a certificate confirming the possession of means of payment in a bank or other financial institution, confirmed with the seal and signature of an authorized employee of that bank or institution, issued at least one month before submitting the application for registration of stay.
Note: If there is a need to clarify or clarify the evidence held by the authority in the case during the proceedings, the foreigner may be called to provide additional documents or to testify confirming the circumstances referred to in the application.
Required fees
No fees are charged for registering residence and issuing a certificate of registration of residence of an EU citizen.

Time
The registration of the stay and the issuance of the certificate of registration of the EU citizen’s stay shall be made immediately.
Information about the issued document
The EU citizen whose stay has been registered is issued with a certificate of registration of the EU citizen’s stay. When collecting the certificate of registration of the EU citizen’s stay, a valid travel document is presented, and if the receipt is made by the applicant’s representative – also the power of attorney to collect the certificate. The EU citizen may present another valid document confirming his identity and citizenship.
Sanctions for breach of the obligation to register the stay of an EU citizen on the territory of Poland
Failure to register stay in the territory of the Republic of Poland is punishable by a fine.
